Comparison of Weight Loss Progress at 3 and 6 Months Between One-Anastomosis Gastric Bypass (OAGB/BAGUA) and Sleeve Gastrectomy

Resumen

Introduction: Bariatric surgery has proven to be an effective treatment for obesity. However, questions remain regarding the comparative efficacy of different procedures for achieving short-term weight loss. In the Latin American context, more local evidence is needed to inform individualised surgical decisions. Objective: To compare the progression of weight-related parameters in patients who underwent sleeve gastrectomy or oneanastomosis gastric bypass (OAGB) during the first three and six months after surgery. Materials and Methods: A prospective study using probabilistic sampling was conducted at the Surgimed Hospital between December 2023 and December 2024. A total of 79 adult patients undergoing bariatric surgery were included: 61 underwent Sleeve Gastrectomy and 18 underwent OAGB. Weight-related indicators, such as weight loss (WL), percentage excess weight loss (EWL), percentage excess BMI loss (EBMIL) and total weight loss (TWL), as well as body mass index (BMI), were evaluated. A comparative statistical analysis was performed at a significance level of p < 0.05. Results: Both procedures resulted in significant reductions in weight and BMI. At six months, OAGB demonstrated greater absolute weight loss (38.5 ± 11.8 kg vs. 31.4 ± 9.9 kg; p = 0.013) and TWL (34.5 ± 5.6% vs. 31.0 ± 5.9%; p = 0.029). Sleeve gastrectomy produced slightly better results in terms of EWL and EBMIL, but these were not statistically significant. No significant differences were found in terms of meeting the criteria for clinical success in terms of weight loss. Conclusion: Both sleeve gastrectomy and OAGB were effective in reducing weight after six months. However, OAGB had a greater impact on absolute weight loss, while Sleeve Gastrectomy was proportionally more efficient in addressing overweight and excess BMI. The procedure selected should be based on an individualised approach, taking into account the patient's clinical characteristics and therapeutic goals. These findings may help to inform the choice of bariatric surgery according to short-term weight loss goals, thereby contributing to more personalised clinical decision-making.
