Introduction: Labial mucocele refers to a process of accumulation of extravasation saliva associated with trauma wrapped in a lining epithelium. They are lesions that can vary in color as translucent blue or mucosal color, fluctuating and fast growing. The site of appearance of the lesion with more incidence is the lower labial mucosa; however, lesions can appear in any area where the minor salivary glands are located. This lesions more frequently affect the young population. Presentation of the case: A 16-years-old patient with no personal or family history of pathology who went with her father to the local health center due to discomfort in the lower lip area. The reason for the appointment is the presence of “a ball in the lower lip that has been there for two years and does not disappear”. The patient mentions that there is no pain or apparent symptoms and does not remember when or how it appeared. A diagnosis of extravasation mucocele was made by histopathological study, and an excisional biopsy was performed under local anesthesia as indicated treatment. The patient had a positive evolution with no recurrence of the lesion. Conclusions: It is determined that in the diagnosis of the pathology the visual inspection is only a presumptive method of first instance since it is necessary the application of the histopathological study to give a definitive diagnosis. The mucocele is a benign soft tissue cyst that is related to the trauma of one of the minor salivary glands, which are located with greater incidence in the lower lip area. Likewise, it is necessary the total exeresis of the lesion to avoid recurrence.
